Glass Replacement Order Form for New Customers

This order form is for new customers only.

If you need glass replacement and you originally purchased your glass from us, please use the Glass Replacement Form for Previous Customers.

Before you proceed, you should have already spoken with us and know whether you will need to send in the full set or just one of the glasses. If you have not spoken with us, please contact us first.

If you need engraving or would like to add engraving we can do this for you as well.

For engraved orders: Please enter your text below. A $20.00 base charge includes two lines on each glass of up to 16 characters on each line. For each additional character we add 75 cents to your cart total. You will have the opportunity to verify/edit your engraving text when you check out.

*Note - engraving is done with the assumption the text will be facing the couple.
