Glass Replacement Order Form for Previous Customers

This order form is for previous customers only.

If you need glass replacement and you did not purchase your glass from us, please use the Glass Replacement Form for New Customers.

Before you proceed, you should have already spoken with us and know whether you will need to send in the full set or just one of the glasses. If you have not spoken with us, please contact us first.

If your glasses were originally engraved, you may choose to add engraving at 50% off our normal prices. Silver or gold fill may also be added at a 50% reduction in price.

Please choose from the options below. When you are done, click on the "Add to Cart" button to proceed to the final checkout page. Glass quantities may be adjusted on the checkout page.

For engraved orders: Please enter your text below. A $12.50 base charge includes two lines on each glass of up to 16 characters on each line. For each additional character we add 75 cents to your cart total. You will have the opportunity to verify/edit your engraving text when you check out.

*Note - engraving is done with the assumption the text will be facing the couple.
